CLAIM FORM INSTRUCTIONS

IMPORTANT: PLEASE READ BEFORE COMPLETING THIS CLAIM FORM

You are a member of the Settlement Class if you are a U.S. resident who had a Minted, Inc. (“Minted”) online account, or provided Minted your name, email address, street address and/or other personal information via email, the Minted website, or other online communications, on or before June 27, 2020.
You can tell if you have a Minted account by attempting to sign in on Minted’s website at www.MintedSettlement.com, or if you received an email correspondence from Minted in May 2020 concerning a data breach.

If you are a member of the class based on the above definition, you may submit a Claim Form. Please complete Sections A, B and C on the form below. Claim Forms must be submitted no later than September 16, 2021.

PAYMENT & BENEFIT OPTIONS

There are two types of claims you can make: (1) an option for payment; and/or (2) an option for Credit Services. You can request both a Payment and Credit Services.

(1) Payments: Class Members who make a claim for Payment receive a payment of approximately $43. The exact amount you will receive depends on how many Class Members participate.
(2) Credit Services: Class Members may also receive free Credit Services provided by Equifax for a period of two years, starting on timely activation by the Class Member. Credit Services include credit monitoring, fraud alerts, and identity restoration services. These services are further detailed in the long Notice available on this website.

Please ensure you provide a current, valid email address with your claim submission. If the email address you include with your Claim Form changes or becomes invalid for any reason, it is your responsibility to provide accurate contact information to the Claims Administrator to receive a payment. When you receive the email notifying you of your Settlement Payment, you will be provided with a number of digital payment options such as debit card, PayPal, or a credit on Amazon.com, to immediately receive your Settlement Payment. At that time, you will also have the option to request that a paper check be mailed to you at the address provided in your Claim Form.
If you elect to receive Credit Services, when benefits are distributed the Claims Administrator will send you information on how to activate your credit monitoring via email. The Claims Administrator will provide you with an activation code which you will use at the Equifax website to activate your Credit Services.
